NDIS Access Assessment
The NDIA (National Disability Insurance Agency) determines your approval to access the NDIS (National Disability Insurance Scheme). An integral part of the process involves the completion of an NDIS Access Request Form. This form draws together the evidence the NDIA needs to assess your eligibility. The application should include supporting proof of your disability, usually from medical professionals and allied health professionals as well as information on the functional implications of your disability i.e., how your condition affects your participation in everyday activities and life. This is where MLEA can assist.
A MLEA occupational therapist can conduct a functional assessment, as supporting evidence to strengthen your application to the NDIS. A functional assessment is a clear way that an occupational therapist can demonstrate and show the NDIA what activities you may require assistance with, what type of assistance you need and how frequently you need it.
To gather this information our MLEA occupational therapist will conduct a face-to-face interview, including assessment, and completion of a written report. This report can be submitted with your competed NDIS Access Request Form for NDIA consideration of your eligibility. MLEA will ensure the report will clearly articulate your current level of functioning and highlight areas and make broad recommendations as to what aspects of your life you may require increased supports or assistive technology. MLEA cannot guarantee your application will be successful nor can we advise that you have enough additional evidence but we can ensue you are provided with a clear access assessment.
